Survey Request Form
This form is designed by the Office of Institutional Research, Planning, and Assessment (IRPA) to provide a mechanism through which requests may be made for technical assistance related to survey needs and requirements. This office will assist by supporting survey development, administration, and analysis.
Please make all survey requests are made at least two weeks prior to the desired implementation date since some survey requests take longer to process.
Requestors are responsible for seeking approval to conduct a survey from relevant ASU departments and personnel, especially the immediate supervisor. Furthermore, all requests must adhere to guidelines set forth by the Family Educational Rights and Privacy Act (FERPA), the ASU Institutional Review Board (IRB), and any other relevant university, state or federal guidelines/regulations. If your survey data and/or results will be shared externally, your research most likely requires review and approval by ASU IRB. Please contact the university’s Office of Research and Sponsored Program (ORSP) for details at orsp@asurams.edu or 229-500-2032. You may also visit ORSP's webpage at https://www.asurams.edu/academic-affairs/orsp/ to determine whether or not your survey needs IRB approval as the IRPA Office is not responsible for IRB applications. If your survey contains any portion of a questionnaire that has been copyrighted or is the intellectual property of another party, your survey may be subject to approval by the external entity. In such instances, you must submit documentation showing copyright approval.
Upon completion of the Survey Request Form, please press the "SUBMIT" button that is located at the bottom of the form. Please note that requests are considered incomplete if there are any blank fields or required supporting documentation is missing. Incomplete requests will not be processed.
